Our services

From lifestyle support to children’s emotional wellbeing and prison healthcare, all built on the same person-centred model.

Integrated Wellbeing Services

Lifestyle behaviours are closely linked, so one service supports people to change several at once, from smoking and weight to activity, alcohol and wellbeing.

Reaching the people who stand to gain the most

Your priority groups are our priority too. We find out where need is greatest, then design support with the people we want to reach.

Targeted where need is greatest

We use public health data, local insight and our Health Equity Assessment Tool to find the communities who stand to gain most. In Gloucestershire, 58% of people taking part in our healthy lifestyles service came from the most deprived communities.

Taken to where people already are

We work through trusted partners in foodbanks, workplaces, housing and community venues. In Sefton, our stop smoking team runs outreach with Peel Ports to reach routine and manual workers on site.

Designed with the people we want to reach

People with lived experience shape how our services look, sound and work. In one recent quarter, 139 lived-experience champions took part in nine co-production projects across ABL.

Healthier communities, built together

Here's what you can expect when you work with us.

Clinical and non-clinical expertise

Our teams bring together clinical and non-clinical professionals, so each service has the skills it needs.

A proven approach

Our approach is built on COM-B, a recognised model of behaviour change. We have refined it since 2009 across more than 20 health and social care systems.

Data at the heart

We evaluate every service continuously, looking at outcomes and at each person’s whole journey. You can see what is working, and we adjust early where something is not.

My Story: Support that starts with a conversation

Every person starts with My Story, a conversation about their journey so far and what they want to change. It builds trust and lets people work on several issues at once or one at a time, at their own pace.

It also cuts the back-and-forth referrals that lead people to give up, which keeps pathways simpler for your system.

One joined-up offer for your residents

When pathways join up, fewer people are lost between services and your teams spend less time chasing referrals.

One front door

A single point of access and triage gets each person to the right level of support from their first contact.

Support that steps up and down

People move between levels of support as their needs change, without a new referral or another assessment.

Built with your partners

We design referral routes and warm handovers with primary care, NHS and community partners. So far we have built more than 150 partnerships and pathways.

Designed with communities, tailored to individuals

Since 2009, community-driven services have been at the heart of what we do. We take services traditionally delivered in hospital into familiar local places, including community centres, churches, mosques and scout huts.

We co-produce services with the communities they serve and focus on what is strong rather than what is wrong. The result is a service that local people recognise as their own.

In Sunderland, blind and visually impaired people helped us redesign how our stop smoking service communicates. Together we produced screen-reader, large print and audio resources, and built referral routes through sight loss organisations people already trust.

Digital support built into the stop smoking pathway

Stubit is ABL’s stop smoking app. It works alongside advisor-led support, giving people progress tracking, practical advice and in-app access to their advisor between appointments.

The Stubit app: smoke-free streak, savings, daily check-ins and in-app advisor chat
  • Free to download
  • In-app access to advisors
  • Personalised advice and resources
  • Progress tracking and check-ins
  • Built-in tools to manage cravings
