Change that lasts starts with listening

We start with what matters to each person, then build the support around them. It's how we help people change habits for good, and how we help you reach the communities that need it most.

Built on behaviour change

Every service we run is designed by in-house clinical specialists and behaviour change experts. Every practitioner is trained to use evidence-based techniques such as Motivational Interviewing.

Our approach is grounded in the COM-B model, helping people build the capability, opportunity and motivation to change. We match the level of support to the level of need, so people facing the biggest barriers get the most help.

We help people build the confidence and support to keep going once they leave us. That's what makes change last.

Of 158 clients in our North West services surveyed 12 months after finishing, 120 said the programme had helped them keep up healthier habits.

My Story: Support that starts with a conversation

Most people we support have more than one thing affecting their health. So we start with one conversation about their life, what matters to them and what's getting in the way, and we agree a plan together.

From there, people can move between our services and local partners without a new referral or another assessment. Different types of support can run side by side. Someone wanting to lose weight after a fall might start strength and balance sessions alongside health coaching, then join a local walking group as their confidence grows.

For you, that means fewer people lost between services, and support that fits how people actually live.

It works from the first appointment. In Nottinghamshire, 94.6% of weight management clients who gave feedback left their session with a personalised goal.

One joined-up offer for your residents

When pathways join up, fewer people are lost between services and your teams spend less time chasing referrals.

We work with your primary care, NHS and community partners to design how people come into a service and move on from it. That can mean a single point of access with triage, step-up and step-down between levels of support, and warm handovers so nobody has to start again. Referral routes are designed with the partners who use them.

We've built more than 150 partnerships and pathways in the areas we work. In Sefton, our stop smoking service works with Mersey Care, Mersey and West Lancs NHS Trust, CGL and Living Well Sefton, and received more than 10,500 referrals in 2025/26.

What working with us looks like

  • Rooted in your communities: we deliver from local venues and partner settings, and go out to the groups who stand to gain most through trusted local partners. In Gloucestershire, 58% of people taking part in our healthy lifestyles service came from the most deprived communities.
  • Shaped by the people who use them: people with lived experience help design, review and improve what we do. We also train volunteers, peer supporters and community representatives so change carries on locally.
  • The right expertise in the room: dietitians, psychologists, physical activity specialists and stop smoking advisers work as one team, with professional leads in each discipline.
  • Safe and well governed: clear pathways and escalation routes, clinical supervision, safeguarding and audit, all overseen by clinical leadership.
  • You’ll always know how it’s going: outcome data is captured as part of everyday delivery, so reporting is accurate and improvements happen fast.
  • Services people know about: experienced central and local marketing teams run campaigns for every service. They build referrals from partners and reach the people who stand to benefit most.

Ready from day one

Recommissioning a service is a significant undertaking, and we treat it that way. Every new contract gets a dedicated mobilisation lead and a plan agreed with you, so you can see progress, risks and milestones as we go. Recruitment, TUPE, training, pathways, systems and launch campaigns are all worked through before go-live. Governance and reporting are in place from the first day.

We've mobilised over 50 population health and mental health services. In one recent mobilisation, that meant transferring more than 125 staff under TUPE and launching a combined stop smoking and lifestyle service in three months.
